The Patient Engagement Learning Collaborative provides an opportunity to connect with peers, share/learn best practices, and receive technical assistance and training from HCCN staff and subject matter experts.
Meetings will focus on building and strengthening healthy relationships with yourself, your colleagues, and the patients and families you care for each day. We will use the Relationship-Based Care™ professional practice model as the framework for the Collaborative.
The Collaborative is intended for health center staff who provide one-on-one assistance to patients outside the standard office visit. However, all interested health center staff are encouraged to attend!
