Positive patient outcomes are achieved when patients are asked about their substance use, engaged with a professional who can provide education and resources, and connected to treatment directly from the primary care setting. In this webinar, you’ll learn how integrated care can provide your patients with better overall health care.
This webinar is presented by CKF Addiction Treatment, which has staff located at Salina Regional Health Center and Stormont Vail Health to provide consultations, care coordination, assessments, and transportation to patients in need of services. CKF has partnered with KDHE on a grant to deliver assessment and treatment services via telehealth to patients referred from primary care. In addition, they have applied for a SAMHSA grant to initiate a Screening, Brief Intervention, and Referral to Treatment (SBIRT) delivery model that utilizes telehealth in a western Kansas hospital/clinic setting.
