“Creating Improved Outcomes Through Patient Engagement”

Serving patients who are invested in their health benefits Kansas safety net clinics in a variety of ways. Engaged patients are more likely to take action to increase their health, improving clinical outcomes. Additionally, patient engagement can affect a clinic’s ability to becoming a Patient Centered Medical Home (PCMH) and in meeting Meaningful Use requirements. Clinics can engage patients in their own care through assisting them to set self-management goals, implementing shared decision making practices, increasing communication about their care, creating a patient portal, and improving patient safety measures.
