With the growing emphasis on patient and performance outcomes and the increasing complexity of providing integrated primary care, behavioral health and oral health services, effective clinical teams are essential to success in community health centers. They can improve patient and provider satisfaction, reduce costs, improve efficiencies and contribute to a positive climate at the center.

This webinar will discuss how to foster high-functioning clinical teams in the patient centered medical home (PCMH) model, including creating dynamic teams, defining team membership and clarifying individual roles and responsibilities.
