Connections Quality Forum: Getting Ready for 2022 UDS

The Forum provides an opportunity for staff at health centers participating in the Health Center Connections HCCN to connect with peers, share/learn best practices, and receive technical assistance and training from HCCN staff and other experts.
In July, we will share the updated Connections CQMs and targets, review changes to UDS CQMs, and discuss how to review and close care gaps.
All interested health center staff are encouraged to attend the Forum! This includes executive/operational leadership, clinical leadership, quality improvement coordinators, care managers, and HIT staff.
